Following the receipt of this form, CHS Alliance will send an invoice and confirmation to your billing contact. 

Important Information to Note:  

  • All conference attendees will register via the conference website (available early March).  
  • All attendees – including staff for exhibits – must have their own conference registration.  
  • Registrations may not be shared, split, or donated.  
  • Discounted registrations beyond those included are available for specific sponsor levels.  
  • If your sponsorship includes exhibit space, you will receive additional information from CHS Alliance as soon as your payment is received.  
  • Sponsorships confirmed after 15th of May are not guaranteed exhibit or slot in agenda space.

  • Accepted file types: png, eps, jpeg, jpg.
    Logo files must be in a PNG, EPS, or JPEG format, minimum 600px x 600px image size and submitted with this form in order to assure placement on the conference registration site and marketing materials.
